How do I get my night blooming cereus to bloom?

I have a night blooming cereus which has gone into urban sprawl--shoots 3+ feet in several directions. It has not been repotted in the two years I've had it and the pot is shallow with little soil. I fertilize it monthly with Miracle Gro. What need I do to get it to bloom? It is in a West facing room near the window. In summer I put it on an Porch where it never gets full sun but lots of reflected light.
